Please complete and submit the attached entry form to HKIHA or MegaIce. Fax/ email the application form are accepted. Completed original entry form must be received at HKIHA office or MegaIce office to complete registration.
All registration will be in first-come-first-served basis. Spaces are limited! Don’t miss the chance to experience the first hand ice hockey skills with HKIHA together with instructions from HKIHA. Please feel free to tell your friends if they will be interested to join. The deadline to apply for the Girls’ Weekend is Oct 21, 2012.
